ULNT2 Neurodynamics Assessment

Radial & Median Nerve Assessment – ULNT2

Radial nerve (C5-T1)

Median Nerve (C5-C7 – lateral cord and C8 and T1 – medial cord)

 

ULNT 2 (Median) Passive Test

Starting position:

  1. Patient฀lies฀supine฀on฀a฀slight฀diagonal฀with฀the฀shoulder฀just฀over฀ the edge of the treatment table to allow for contact with the therapist’s thigh.
  2. Therapist฀stands฀near฀the฀patient’s฀shoulder฀and฀uses฀thigh฀to฀ carefully depress the shoulder girdle.
  3. Therapist’s฀right฀hand฀cradles฀the฀patient’s฀left฀elbow฀and฀the฀left฀ hand controls the patient’s wrist and hand.
  4. Patient’s฀arm฀is฀in฀approximately฀10฀degrees฀of฀abduction.

Movement sequence:

  1. Elbow฀extension฀and฀then฀whole-arm฀external฀rotation. •฀ Wrist฀and฀inger฀extension.
  2. Glenohumeral฀abduction฀is฀then฀added฀if฀necessary.

Structural differentiation:

Based฀on฀where฀the฀symptoms฀(if฀any)฀are฀located. •฀ Same฀as฀for฀ULNT฀1.

ULNT 2 (Radial) Passive Test

  1. Elbow฀extension฀and฀then฀whole฀arm฀internal฀rotation.
  2. Wrist฀and฀inger฀lexion฀(may฀also฀add฀wrist฀ulnar฀deviation฀and฀thumb flexion).
  3. Glenohumeral฀abduction฀is฀then฀added฀if฀necessary.

Structural differentiation:

  1. Based฀on฀where฀the฀symptoms฀(if฀any)฀are฀located.
  2. Same฀as฀for฀ULNT฀1.

Palpable areas:

  1. Mid humerus
  2. Radial sensory nerve on the lateral aspect of the forearm

Common entrapments / syndromes

  1. De Quervain’s tenosynovitis
  2. Supinator muscle (tennis elbow)
  3. Post humeral fracture pain
  4. C5-6 root syndromes
